Sometimes it is because the agent never answered their phone (which is the session being referred to), sometimes it will be because the network connection is so bad that while they may have answered the phone, the system does not KNOW they answered the phone.
If you are not sure, you can dial in to the agent's "session" directly and see if you can talk to them. If you can, there is a deeper problem. If you can not, then there is a problem with the call to the agent during login.
