I have vidial 2.2.0b8 and vtiger510 with the vici patch applied. I have an agent that has figured out if he clicks the number twice fast enough, he can call the customer twice at once!
1) Agent logs into vicidial in a outbound manual dialing only enabled campaign
2) Agent logs into a vtiger
3) Agent clicks the lead's number twice
4) lead's cellphone voicemail picks up
5) Agent starts leaving message
6) lead answers his cell phone
Both Agent and lead can head the voicemail prompts.

My solution: Fire the agent
There is only so much we can do within the Vtiger interface, and the click-to-dial is pretty basic. I'm sure we could try adding some extra Javascript to deactivate the function once it's clicked, but that would have to be a paid or contributed add-on since we are working on a lot of paid development at this time.

LOL, my response when he showed me, "well don't do that!"
Anyway, I just wanted to pass on that it was possible =)
