We have more year with Vicidial but now we need to have a panel for Agent to ask the client with some questions.
The reason is to create a script for AGENTIT where to interview with some questions the person that we call.

To be more clearly, we need to make some questions as: true, false or others to discussion and to go Next questions Next

You need custom webform.
Build the from and processing backend and provide the link to in in campaign settings. -> Web form.

if you are not sure how to build it - you will need to hire someone to do that for you, but implementation in to vici is fairly straight forward.

You could also use custom list fields, then everything will stay within Vicidial.

You need custom system to capture your webform data and store it.
Then you need custom system to view/generate report.

Have you read the Vicidial Manager Manual? It explains how Custom List Fields works. Basically, they will show up in the agent screen in the FORM tab, that will show up once you have configured them on your system.

Thank you, but to use Custom List Fields I have implement and in use, but is not the best way for me, Why I ask who is the best way.

The newer free version of the Manager's Manual likely covers custom fields. EFLO.net (paid version is better, though, well worth the money).

We have many clients who use Lime Survey with Tokens for interviews with Vicidial. But almost any third-party interview/survey software can be linked.

2) As you are obviously new here, I have some suggestions to help us all help you:

When you post, please post your entire configuration including (but not limited to) your installation method and vicidial version with build.

This IS a requirement for posting along with reading the stickies (at the top of each forum) and the manager's manual (available on EFLO.net, both free and paid versions)

You should also post: Asterisk version, telephony hardware (model number is helpful here), cluster information if you have one, and whether any other software is installed in the box. If your installation method is "from scratch" you must post your operating system and should also post the .iso version from which you installed your original operating system. If your installation is "Hosted" list the site name of the host.

If this is a "Cloud" or "Virtual" server, please note the technology involved along with the version of that techology (ie: VMware Server Version 2.0.2). If it is not, merely stating the Motherboard model # and CPU would be helpful.

3) I thought they were labeled, but honestly have not looked. If you're using an older version of Vicidal, you may want to upgrade (or load a newer version into a virtual sandbox to test this theory).

4) Look at other reports. check all the report options ...

You should list the version of the .iso you used. (Vicibox 3.1.0 vs 6.0.4 makes a BIG difference). You should also list the full asterisk version (1.4 covers many sub-builds, each of which may have different idiosyncrasies ... plus the "-vici" at the end can make a big difference sometimes!)

If I use a LimeSurvey all data to export I will try from LIMESURVEY? All customs fields that I have add in limesurvey script, I will export from limesurvey admin panel?

Vicibox 4.0.2 was a little buggy, but not bad. Vicidial 2.6 is very old. You may want to upgrade ... or even reinstall with Vicibox 7.0.2

Of course, if it's working for you, don't mess with it, LOL. But consider a fresh install for testing so you can see how it works now. After all, a virtual vicidial is free to install and works for a single agent for testing.

Exporting works in both packages. So I have no idea what you are trying to ask. I also don't know why you would have the data in both applications or what your goal is when you export. Not really possible for me to form an opinion on how you can achieve a goal without any understanding of the situation at all.

All I have to work with is that you want to export data ... which can be done from either application.
