Inbound call immediately dropped

My problem is that sometimes when an agent answers an inbound call, the system seems hangup the call immediately.
It is so hard for me to replicate the current problem but I do get the logs.
I can assure that it is not from the customer/PSTN side that dropped.

How can I prevent for this to happen? Customers are wondering why their calls are being dropped when an agent pick-up the call.
Any explanation or help from you guys is much appreciated.

How can I prevent for this to happen? Customers are wondering why their calls are being dropped when an agent pick-up the call.
