Do your agents use the LOGOUT link to close their sessions and hang up their phones every time they close out a session?

i had exactly same problem and i found out that it was because one of tables crashed ( some live_channels sorry i dont remember proper name ) . I repaired that table and problem was solved .
Do you get any sql or asterisk error after logon ?

Not usually, but when this happens I make the agents
to check the conference members view and close anything strange
there.

login to mysql client and execute
repair table <tablename>.

You can also use check table <tablename> to check if table is fine . I use phpmyadmin for most of this stuff .
Btw if you have any sql problems then vicidial mostly throws them on login screen( you get lot of mails ) so you can check tablename there mostly .
