AMD problem

I've enabled AMD on a VDAD campaign and had it pass detected AMs to a voicerecording extension. Now some agents receive apparently a live call transfer (or maybe AM) but there is no call transferred to the vici conference. Data shows up on the CRM, but the agent is all alone in the vicidial conference session. When I check asterisk CLI logs of the * server where the agent's phone is connected, the phone number that popped up on the CRM wasnt even dialed on the * server. below are the extensions.conf entries i used w/ AMD

what could be causing those fake transfers? am i using the wrong loadbalance agi?

Asterisk CLI output?

agiout logfile output for the transfer AGI?
