What is going on?! Multiple calls in conf, no reason why

No matter WHAT we do - we KEEP getting multiple calls into one conference, it's gotten to the point where the dialer is unusable.

I have a X100P card.
I'm using GSM.
I'm using VoIP, IAX non-trunked.

10 agents, dialing anywhere from ratio/4.0 to ad.avg/7.0.

Hardware is pentium 945, 1.5gb ram... asterisk 1.2.18

SQL queries state single instance of each agent in the web sessions table.

Agents are logging in and and out correctly, I watched and made SURE of this.

What the heck is going on? Am I missing something?

and are you recording all the calls ?

is this a multi-server setup? either you have a misconfigured extensions.conf and meetme.conf or you're agents are not logging out properly. i would assume that you're agents are not logging out properly just closing their browser thinking that this automatically logs them out but it doesn't.

how many vicidial_conferences entries are in your database? any duplicates?

I see this all the time when people dont log out properly and the phone stays connected (on a second line) then you get crosstalk when someone else logs in.

next time it happens go into the asterisk CLI and do a show channels see if there are multiple extensions in the same meetme conference.
