The iax.conf that gets autogenerated adds a few extra lines to what was manually inputted into the carrier (i.e. host=dynamic). I spent a few days of troubleshooting before I finally came across a page that mentions that if there are two different hosts specified it causes a malfunction. I looked manually in iax.conf and deleted the auto added parts of iax-vicidial.conf and lo and behold I could make calls!
I just thought I would mention this because it caused me hours of lost time spent troubleshooting.
