I have enabled custom fields and also added fields as per my need. But im not being able to see the fields in the agent screen when dialing(manual/Auto). How to do it?

You'll find the custom fields on the FORM tab of the agent client.
You can optionally set the campaign/ingroups to automatically launch that screen when agents receive a call by setting the Get Call Launch value to FORM on the appropriate campaigns and/or ingroups.
