agent does not connect with dialed number

When I click on the login button, this is what happens. My phone rings, and I pick up. I just hear dead air. here is what the CLI shows.

After this the dialer calls the number from the hopper and that phone rings. I answer that, but it does not put the agent and the called party together.

also with no calls in progress, it still shows live channels.

Can you post CLI output from when the customer number is called?

I think this whole install was screwed up, so i have reinstalled. The dead air was caused because I had a zap device and no zaptel configuration, but asterisk ran like crap so i have formatted and reinstalled everything.
