Good Day: I hope all is well. I appreciate you all putting in beneficial comments to help us all.
I have installed GoAutoDial.iso, downloaded it form the official website.
My server hardware and software specs are as below:

Hard Drive: Sata 1 TB

I'm facing some severe and a couple of minor issues and would like to have suggestions to resolve these. Please see below the issues:
1) Leave 3-way Feature Doesn't work:
When my rep clicks the transfer button on Agents screen sometimes "dial with the Customer" button on transfer windows dies (unclickable). If we somehow transfer the customer to the closer, and we press leave 3-way button on the transfer window, we may loose the close, or the customer or both. That's why my rep have to put it in mute and wait until closer ends the call.
2) Unable TO Write Frame To Channel At Extension ...............
some Times we also see this message on CLI when my agent dial manual numbers from there account. What is this and how it can be fixed?

3) Please See Below Some Other CLI Errors I usually see more than 10000 times a shift:
WARNING[28431]: file.c:1297 waitstream_core: Unexpected control subclass '-1'
WARNING[26360]: rtp.c:953 ast_rtcp_read: RTCP Read too short
ERROR[30166]: utils.c:967 ast_carefulwrite: write() returned error: Broken pipe

Have you watched the Load during these issues? Ordinarily these buttons work all the time or Don't work all the time (if there's a bug). So having them work sometimes but not others is generally a sign that you have load issues with your server. Does the problem occur during the busiest portion of your day?

If not, it may be worth the effort to upgrade to Vicibox 4 and see if there is an improvement (not to mention the cool new toys like Quality Control ... in the later versions ...). You can also just upgrade in place to get the later version if you like the CentOS system presently installed. But remember that GoAutoDial has that pretty screen you will lose.

That version is over a year and a half old, we have added some fixes for this issue since then. I would suggest that you upgrade.

It happens all the time. Even if we have 5 or 20 reps logged into it.
Can you please suggest how can I upgrade without hurting my DB?

The SVN system will acquire the source code (that's all it does, it makes no changes to your system ... it's just a downloader ...). After it has downloaded, read the UPGRADE document included in the source code. It contains both installation instruction (for the code/software) and upgrade instructions for the database.

Backup is highly recommended.

You can apply the latest GoAutoDial CE 2.1 agent UI fix here: http://goautodial.org/issues/27. See if that fixes your issue.

How can I check if the call has been successfully left or disconnected form both ends?
