Dial with Customer Issue

Hi,

We are having lots of problem with 3way calls.
1. I ve set Transfer-Conf Number 1 as the number of the TPV. Agent is now is engaged in a call and ready to 3way the customer to TPV, agent now will press trans-call then D1. after D1 number is not showing. any thoughts about this? this happens once in a while i just tell them to logout login after the call
2. Agent is engaged in a call and ready to call TPV. press trans-call then D1 to show TPV number, then dial with customer is not clickable. I am lost here to why it is not clickable
3. same process as #2 but now it is clickable, after pressing dial with customer, the call is not pushing at all,

Is this system upgraded from an earlier version, or never changed since install? New problem on old system or Fresh install with this problem being present since the build-out? Was the database brought from an earlier version and "upgraded" to meet the new server specs?

Server load during an occurrence? Check for packet drops to agent from server (not likely, but still possible ...)?
