Problem with 3-way calling.

I called up one person then did a call with target to another person and the 3-way call went ok, until I tried to release them into speaking with each other.
At that point the two parties reported that the system told them about an invalid extension.
The agent login reports VERSION: 2.0.105 BUILD: 61221-1212
And log/asterisk/full shows:
