Vicidial not detecting connected call on manual dial

Hi all,
We are having an issue with a new install. If an agent is logged into a campaign and decided to manually dial a number or do a scheduled call back, the system does not detect the live call.
The status indicator on the top remains at: Calling XXX-XXX-XXXX UID: M703XXX Waiting for Ring .... 1 Seconds
I would assume that it isn't detecting the channel or the AGI calllog is failing at some point. Has anyone experienced this?
