Transfer Conf not available on Manual Dials

So I have:
I am working on a cluster that has 1 DB, 1 Web, 3 Dialers:
VERSION: 2.8-440a
BUILD: 140521-2101
Version: 2.8b0.5
SVN Version: 2122
DB Schema Version: 1378
Installed via VICIbox v.5.0.3 Standard 64 bit (made on 5/30/2014 from vicibox)

And my agents can make calls, transfer-conf and dial with customer, no issues at all.
When they make a callback or place a manual dial all that happens is "Waiting for Ring......"
but never does it show the call is connected to allow Transfer-Conf... even though both parties ARE connected and talking, ideas?

look at the asterisk log when this happens

I was just able to resolve this issue myself. I do have a question and i need you to post something

First does your asterisk version under servers match the asterisk version that you have?

Also please post your dial entry and your account entry under carriers.
