Help with Surveys -Survey is setup but does not run

I am using VERSION: 2.10-446a
BUILD: 140625-1931

Setting up a survey seems pretty simple. I duplicated a working campaign. uploaded the audio file Went in set 1 and 8 as my dtmf digits
and submitted. selected agent transfer as survey method and deleted the 3 and 4 digit info as I had no 3 or 4 digit. I then put a list with my a couple of my phone numbers in it to test it. It is simply connected directly to the agent and did not play the recording. Is there somewhere else in the campaign I have to activate the survey?

It is actually is connecting now but immediately hanging up and does not give play a recording or give a chance to press 1.

Even me facing same problem call directly getting connected to live agent no audio played from survey file

i tried to check agiout log file can't see my number dialed in the list i can see the other campaign log files but not survey campaign can you help me

I tried One at a time with AMD 8366 the calls are going blank on both end and with 8373 calls are directly getting connected with agents without playing audio file

Any update on getting the audio to play. I had given up and not notice the response until now.
I would be willing to pay to have somebody help me get this to work.

can you post the asterisk cli log without any debug enabled.

make sure you have converted your voicefile in right format
ie: 16 bit Mono 8k PCM .wav
if not use the below link to covert a file

make sure the voice file is located in /var/lib/asterisk/sounds
