This problem is happening for both Incoming and outgoing calls.
Post your dialplan entry (Admin->Carriers). Modify any IP/User/Secret/Password information, of course.
If you can, post an asterisk CLI entry from the moment this occurs. A test call, though, with no other traffic (not 3000 lines of unrelated data.. just one call!)
