Agent Status showing dead in real time screen

Hi all,
We installed vicidial | VERSION: 2.11-452c | BUILD: 150302-0944
While agent is on call (Inbound/Outbound) more than 3 minutes he will get call hangup option in right upper corner of the screen and in real time screen agent status is showing DEAD but agent is continuously talking to customer it is not happening for all our agents
This problem is happening for both Incoming and outgoing calls.
