Add new fields to customer info form for agent

Hi,
I have couple of changes to do with my current vicidial installed. First is that I need to add fields to customer info form where agent add information about the customer and I also need these fields to be shown on report. So please any guidance regarding this will really be appreciated. Till now I have came to know that there is a custom field option in the admin lists panel, I have added new and modified old fields but i couldn't see any effect on the customer info form.

Second think I want is that whenever a lead is generated, an notification or email is sent to admin group with the details of the lead. He wil be adding more information to it so some fields are required for the admin too. Kindly guide me as I am totally new to vicidial.

Custom fields are accessible to agents in the Form tab, not the main tab. These fields are accessible also when you search leads in the Admin and in the export report.

I believe other people have used the Dispo Call URL in the campaign settings to call a page to send emails. If you search this forum you will find discussions on this.

Thanks for your response. I went to the form tab, clicked on refresh it gave me following error
ERROR: no custom list fields table


Can you guide on this? Am I using old version?

The form tab is only accessible when the agent is on a live call and the lead must be from a list that has the custom fields created.
