Unable to warm transfer

As the title says, I am unable to do warm transfers within the Vicidial phone panel. I can select park call, then select transfer - conf and enter my number into the number to call field, but the dial with customer button and park customer dial buttons never become select able. Eventually I will get a dial timeout error.
I am running VicidialVERSION: 2.12-552a BUILD: 160429-0835 | Asterisk 11.22 | Single Cloud Based Server | No Digium/Sangoma Hardware | No Extra Software After Installation
Any input would be greatly appreciated. I have both manuals so I'm not sure if I am missing a user configuration step or not.

Still looking for a solution to this if anyone can assist please.. skimmed both manuals and haven't come across a setting I may have missed when setting up my users. Maybe I'm just not understanding the correct process of a warm transfer.. Any assistance would be greatly appreciated
Update, it looks like I can press the dial with customer and park customer dial once I enter the DID.. the caller is placed on hold, but I am unable to hear the transferring line ringing on my end. Eventually I get a dial timeout error.
I am following the following process per the agent manual:
1. Click the “TRANSFER-CONF” button on the left side agent screen
2. Enter the DID in the “NUMBER TO CALL” field
3. Press the “PARK CUSTOMER DIAL” button (below & to the right). The customer will
immediately be put on hold (Park) while the dialer places the call. Once the call completes
to the phone number, you can talk to the 3rd party as long as you wish.
At this point, the caller is parked... and as the agent, I don't hear the transferring line I am inputting ringing, and I eventually am given a timeout error.

bump... still unable to warm transfer using the Vicidial phone panel. I am out of ideas and would really appreciate some assistance if possible.
