Agent interface buttons grey no live call but on a live call

Usually I would chalk this issue up to internet connection. When calling certain numbers (random) no live call ever appears so the whole interface is grey and un-useable yet the customer is connected and both parties are talking fine. Its like the dialer is not getting back the correct response from the carrier to acknowledge it is in a call.
Looking at the sip packets it seems like they are identical but I am no expert , I see the Invite and ACK on both . I can post that information if you think it will help. Just figured id see if this was a common thing for anyone else. Maybe a setting in my carriers. I really don't run into many issues, we have been going strong over 5 years now. What really sucks is its not like it passes back some error i can parse in the dial plan and fail over. the call seems to complete as normal.
