Limesurvey / Lead Search

We've been using Vici and Limesurvey for quite some time. We've created dozens of campaigns and surveys that have all worked great together. We recently created a campaign and survey that has an interesting quirk. When a call is placed, the survey pops up and populates perfectly on all calls. However, if we do a Lead Search and select an account, when we go back over to the survey and refresh it, it is blank. No survey and no data populated - just a blank webpage.
We use the Lead Search functionality on all other surveys and it works fine. Often, we have to 'refresh' the survey screen, but then everything shows up. It's only on this survey that this isn't working. We believe we checked all of the script options in the Campaign, List, DID and Script in Vici to make sure it matches other scripts. Any thoughts as to why this is happening?
