survey campaign direct to callmenu not working

I have setup vicidial campaign with survey option and directed to call menu, details as below
set campaign route extension to 8366
survey method is set to callmenu
Added one remote agent to start a campaign
Added list and leads to test
calls are getting dialed
Issue:
when calls are answered its not transferred to call menu but are hang up immediately
Any idea what can be the issue and how can i fix it
