Hey we are experiencing a agent ui issue which, after the call and the agent will click dispo but no dispo screen will appear and the ui will refresh ready for new number to be dialed but it will be the same again for the next number until we logout and login to fixed the problem. I check the apache logs and it was fine. I dont where I can fix for this. I try searching through the web.
This is on Server 2.

Have you checked the campaign settings for
Disable Dispo Screen
Make sure you have it set correctly.

It was set correctly "Dispo_Enabled". We have dispo screen but its frequently disappear from time to time. Agent need to login and logout in order to make it work again.

is the server overloaded? does this happen in any sort of pattern? (certain agents, but not others, only one server ...? only one campaign? only one location for the agents?)

Server is not loaded.
Pattern? No, it just happen time to time. Its comes back at random minute
Two agent is only dialing on it. They experienced it both.
They dial two campaign at different shifts and they experience it on both campaigns.
Location? If you mean they physical location, Yes. They're also under the same network.

In the past strange things have happened based on random keystrokes of agents whose screens were not expected to receive input at that moment. If the agent hit "return", they'd get another client in their session. Even though there was no visible field to accept the return keystroke.
That sort of thing is rare, but does happen. I'd start with an upgrade, before anything else. Since upgrade is free, and if it solves your problem you no longer care what was causing it. lol
Did you build server two at its present state or was it upgraded from an earlier version? Did you import any data or files after building it?

It was a fresh install. Not an updgrade. Yes some leads to dial it with. but not more than 10000.
System Load Average: 0.21 0.37 0.29 M - Here's the current server load as of this reply. (3 Manual Dialing Agents)
Server 2 Specs is the specs inquiry that I ask you these past few weeks which is -
i3-3.10GHz 2Core - 4Logical | 4GB DDR 3 RAM Non ECC | 1 Integrated LAN Gigabit
Do you think this is a network issue? Will adding a Dedicated NIC card help the performance of server2?

Manual dialing ... do the agent sometimes use the manual dial override field?

Are you sure? Cuz that would explain it comfortably. Perhaps some log checking on the differences between the calls that work properly and those that don't is in order. Pick one of each and compare ... and hope that's it cuz if it is, you're done.

do you want this problem to crop up occasionally? cuz if you don't reboot nightly ... it'll be back again, eventually.

If you don't wish to reboot your server nightly, you can do service restart on apache2 service and asterisk to clear anything that might be stuck.

OR perhaps it will clear "some" stuff but not everything. We've had clients who reloaded mysql, apache and asterisk and still had issues over time. Maybe MORE time, but still had issues.
Reboot is free.
We cut 75% of our "Server is broken!" support calls out by rebooting servers. Most of the rest were NOT broken servers, but client configuration issues. (We have ... a lot of Vicidial servers!)
Just sayin' ...
