Caller ID name in comments field

I've seen this on a few servers and don't know where it's coming from. When an inbound call is received the caller ID name is saved in the comments field. When you use audited comments the next time you call back to this lead you'll get popup with that name in the comment which is totally useless because it's not a real comment made by an agent.
How do i disable this?
