Survey to Call Menu > Ingroup is not showing UID as phone

I have a survey campaign setup.
I created an outbound campaign with Survey, when somebody press 1, it directs him to a Call menu, when someone press 1 on Call me, it directs them to an In-Group of my agents, but my agents are not seeing customers number on their screen, they are seeing UID.
Outbound Campaign> Survey> Call Menu > In-Group > Agents
This is how my dialplan is setup:
