Simultaneous Inbound

We are having an instance where sometimes we get 2 inbound calls at the same time. One goes to 1 agent who gets dead air, the other goes to another agent who gets the call. The agent who gets the call sees the agents who didnt get the call as the Fronter, however we dont have an transfer settings that would allow an agent to transfer an inbound call to another agent.
On DID log of calls you can see an example below with 2 differnt log entries at the exact same time. When I search the lead I see 2 Inbound Calls, with 2 separate recordings. The call with dead air has a recording 2-3 seconds before the call that gets to an agent. Is there some setting I might have incorrect that could cause this?
