All installation and configuration problems and questions

Often when i place a manual call my call is connected and i can talk to the called party, however the agent screen does not indicate a live call is connected and i therefor cannot hangup the call or conference / transfer the call. My only option is the logout to end the call. This has happened with 2 different carriers.
vicidial Version 2.14-718a SVN 3893 DB schema version 1722
I have a 4 machine cluster
Database/ WEB / Dialer machine dual: quad core Xeon 2.67 GHz 48GB mem / Dialer dual: dual core Xeon 2.80 GHz 8GB mem / dialer core i5 3.0 GHz 4GB mem / dialer core-2 duo 2.93GHz 3GB mem
.. all machines have 2 nic's.. one internal and 1 external for traffic.

The manual dial page has multiple fields you can use to generate a manually dialed call. Which field(s) were you using specifically?
The Asterisk Version being incorrect during the execution of "install.pl" can also cause the wrong conf files (especially sip.conf) to be used and this can break several functions, such as some of the buttons on the transfer-conf and others in the agent screen.
Please include your Installer with Full Version in your posts. In cases like this the installation method may come into play directly, and instructions are specific to the installer.

Thank you for your input. I had been testing calls and transfers and had tried using both locations on the agent web client (dial code & phone number) as well as the dial override fields. I was receiving strange results as my test numbers were either cell phones and voip phones and i was often forwarded to dummy voice mail. By creating a test file rather than using manual dial many of the problems have disappeared. I can now continue testing
