No one is in your session..

I am getting the 'No One is in your session' most times the agent login. Very often too the message that 'You are the only person in this conference ' does not play.

But, sometime, I am able to login and it works fine. I cannot figure the problem, (not that I will know exactly how to trouble shoot any Vicidial issues ).
I am posting the following as per similar earlier problems in the forum.This one is when the agent logged in successfully. Also , the transfer conference gets frozen often and we cannot type the number to be conferenced in the middle of typing the number. DTMF also passes sporadically, sometimes good, sometimes not. The calls transferred with customer gets cut off in the middle too.
Kindly help out with these problems and how to approach and rectify them.

When I run the script from command with the --debug there was no output at all. But I sometimes get the below : -

One problem at a time. For the "no call in this session" please document every step taken, look at the Asterisk CLI and post the live channels data for one specific instance when it is happening.

Is there any pattern to the failures at all?

When i logged in one agent phone , it rang and then i picked up the phone. There was no message 'You are the only person in this conference" played. There is no out put in the asterisk cli [screen -x asterisk]. I am getting inbounds constantly and only these AGI scripts are running.

Then the agent screen says 'No one is in your session' and 'call agent again'. If I hang the phine and use 'call agent again' it rings again and the process is repeated. If i press RESUME tab, the screen shows the inbound color band and callerID , but no voice. After one such call the phone start beeping (engaged tone) till I log out.
The live channels output when the agent phone logged in is :-

I went through the same problem when load on server is expectidely high, check the load on server, when this is happening.

The troubles are resolved. Though I do now know what I had done wrong , re-checking the configs and database entries and the phones and the vicidial users sorted it all out.
