Loading Leads Problem

Vicidial could not load leads and it show this error whenever the new leads is submit
Warning: fopen(/tmp/vicidial_temp_file.txt) [function.fopen]: failed to open stream: No such file or directory in /usr/src/astguiclient/www/vicidial/new_listloader_superL.php on line 786

Warning: fgets(): supplied argument is not a valid stream resource in /usr/src/astguiclient/www/vicidial/new_listloader_superL.php on line 790
ERROR: The file does not have the required number of fields to process it.
please can anybody tell me what is wrong,
I'm using vicidai 2.0.4

When you are loading leads (on this page: http://192.168.10.74/vicidial/new_listloader_superL.php) there is a ( ? ) next to the browse button, this is a help button that will list the lead's structure requirements for your build of vicidial.

There is also information in the manager's manual for exactly how to load (and structure) leads. It is best to load one or two leads first until you get the field list correct and see how it loads before loading an entire massive list.

From my latest machine: (LIST LOADER-VERSION: 2.0.4/BUILD: 70510-1518)
Vendor Lead Code - shows up in the Vendor ID field of the GUI
Source Code - internal use only for admins and DBAs
List ID - the list number that these leads will show up under
Phone Code - the prefix for the phone number - 1 for US, 01144 for UK, 01161 for AUS, etc
Phone Number - must be at least 8 digits long
Title - title of the customer - Mr. Ms. Mrs, etc...
First Name
Middle Initial
Last Name
Address Line 1
Address Line 2
Address Line 3
City
State - limited to 2 characters
Province
Postal Code
Country
Gender
Date of Birth
Alternate Phone Number
Email Address
Security Phrase
Comments

Create a tab delimited file with excel, with those exact fields, and then import the file. Blank entries in fields are fine, but all fields must be there.
