Has anyone installed VICIDIAL on a production level PIAF box? And does Vtiger integrate with this system?

We usually don't recommend any single-server VICIDIAL/FreePBX system due to the inefficiencies of the dialplan within FreePBX.

As for Vtiger, we have been adding a lot of integration options in the SVN trunk codebase including user synchronization and better lead/account/vendor searching within Vtiger from VICIDIAL. We are working on more elements as well in the near future.
