Hide fields in the load leads form

LIST LOADER 4th Gen-
I would like to know how I can hide some fields from the load leads form.
I want to hide the following fields:
USA-Canada Check:
Lead Time Zone Lookup:
State Abbreviation Lookup:
Required Phone Number Length:
