Mutiple calls coming at the same time

We are using VididalBOX

VERSION: 2.0.5-173
BUILD: 90320-0424
Asterisk 1.4.21.2


with ubuntu 2.6.24-26-server

agents are randomly getting two calls at the same time in auto dial mode

has anyone faced this issue before

please letme me know if any additional information to solve this issue

Improper logouts.

Loaded server.

and wrong sub forum.
