CallerID Restricted on Outbound Call

Can anyone tell me why the Caller ID would be popping up restricted when making outbound calls? I have the Campaign CallerID set to the number I would like it to broadcast. Any help would be appreciated.

Has your carrier approved the number you are sending?

shall we assume that these calls are auto-dialed? (not direct-dialed from the sip phone)

shall we further assume that you have NOT modified your dial plan entry in some way (or have other software on the box)?

perhaps you should post your dial plan entry for this carrier :)
