Problem with call transfer

We are using following version of Vicidial
VERSION: 2.4-309a
BUILD: 110430-1642
With this version I am having following problems

a) When a agent does call transfer , In the case the number where the call is transferred is free then it works. If the number is engaged , then the agent is not able to recall and the line gets hanged.

b) When a agent does call transfer , In the case the number where the call is transferred is free then the call gets disconnected when the agent transfers the call and connects two .

Any configuration issue that may lead to this problem ?

List the EXACT steps you use for "agent does call transfer".

Have you read the manager's manual regarding call transfers?
