Blank Screen on GoAutoDial? No name and phone number appears

Hi Guys

We are experiencing issues where the customer name and phone number does not appear in agent screen. It appears as "BLANK SCREEN" Although in the database we have the customer name and phone number. Any thought why??

Do you know if lead_id is only OK to capture 9 digits? Our record has gone past 10 million records just today and it seems that it is NOT showing the info. Please help... Thanks.
