Tone appears as busy fast - Vicidial

When the agent is speaking, a tone appears as busy fast. It does not stop until the session closes.
The VERSION: 2.14-685a Vicidial.

Please, help me.

Post asterisk cli from a single call which has this problem. (Not 3000 lines of unrelated code, of course, just ONE call from beginning to end.)

Also: Where are you in the Vicidial Manager's Manual? If you started at page one and didn't skip anything, you'd likely end up with a fully functional system without this problem. Consider starting over (free, right?)

How did you install your server? (Please include a link to your instructions and the full name/version of the .iso installer you began with.)
