how i can make a form in my campaing

I am using the vicidail and I'm running a campaign in which I need a script style, form, or rather the hood I'm running is a survey type form. How can I make a form for this campaign

If you want a form to "Pop" when an agent gets a call, that's already built into vicidial.

Modifying the content of the form and where the data goes ... well, I should ask first if you have the manager's manual. Do you have a PHP guy handy? After you have a form "pop" with the appropriate database information in it from vicidial's built-in system ... the sky's the limit on what you do with the data.
