this would not be a "vicidial" question. we'd love to help, honestly, but this is a question of the call being rejected:
-- Called 40016777/0022720133150
-- Got SIP response 503 "Service Unavailable" back from 203.196.128.58
-- Called 40016777/0018003229266
[Oct 15 20:20:04] WARNING[3074]: chan_sip.c:12177 handle_response_invite: Received response: "Forbidden" from '"V1015202004000363738" <sip:91403@203.196.128.58>;tag=as497aac90'
-- SIP/40016777-09c0af78 is circuit-busy
by your provider. you could try providing this information (and your sip context as well) to your provider and asking why they are rejecting the call.

service unavailable and Forbidden ordinarily means that your provider didn't like your call. WHY they didn't like the call would be something you'd have to ask your provider. as a rule that's not something you can get from the technology codes being passed. it could be your caller id, the number you called, you may not have permission to call premium numbers, just about anything.

Williamconley is right. The best way to do this is to troubleshoot with your provider on the other end monitoring your calls. This will save you lots of headaches and sleepless nights. :D
