Vicidial not creating new records in vtigercrm

Hi.
The test pop up all proper info. Sink seemed to have worked for users.
But vtigercrm not creating new records. No records are being created can somebody please help with this?

Did you read the VTIGER.txt document?

Did you apply the vtiger patch?
