Agent login to conference with other agent and caller in it

Okay I am getting some weird problems with a vicibox install that is being used for predictive dialing.

Sometimes an agent will get kicked off the system while waiting for a call, then the agent logs back in and directly into a conference with another agent already logged into it. Sometimes that agent already has a customer on the phone. I can't figure out why this is happening it seems to come and go without any reason. It will happen across multiple campaigns and within the same campaigns.

Another problem I am getting is sometimes an agent will be logged in waiting for a call and 2 calls will come in at the same time. So for example I had an agent a moment ago receive 2 answering machines at the same time, I had the agent log out and log back in and it seems to be fine but this is happening from time to time.

Anyone else have these odd problems? Is it something with my configuration? Any advice would be appreciated.

1. The cpu is around 25%, 1.5 Gig memory free
2. 28 agents
3. We have 3-4 campaigns running most are set to RATIO = 3, 1 is set to ADAPT_AVERAGE
4. 1 Server

What is the loadavg when this happens? (not looking for CPU %)

I have just upgraded to 2.0.5.1rc1 from 2.0.4 and I am getting agents crossed into same conf room. Seems like if I login as an agent I will get into session 8600051 then if I log out and back in again I get conf # 8600198. each time i log in it alternates between these two conferences. (this is with no other agents logged in and no load) when I get session 8600198 I remain logged in and have another agent try to login. they will get 8600198 as well and cross with my campaign. I used the extensions.conf that was supplied with the upgrade and just moved my customizations in with it. everything else seems to work fine. I would be willing to pay for Matt or somone with similar fimilarity with vicidial to look at this as it just lost us a deal and I want to try to resolve it quickly. I have looked for Duplicate conf #'s and see none. also loadavg is 0 at the time.
