VICIBox3.1.9 Cluster Install Problems - Leads Loading

Hi,
Cluster info :
DB Server
HP Proliant DL180 G6 Dual XEON quad core 16 GB 1 TB
Kernel : 2.6.34.7-0.7-default
Installed Using : vicibox3.1.9 64 Bit ISO
Install Reference : VICIBox Cluster and Walkthrough Guide
Above server is dedicated database only
Web/Telephony server
HP Proliant DL180 G6 Dual XEON quad core 16 GB 1TB
Dual Digium TE420P (total of 8 spans)
Kernel : 2.6.34.7-0.7-default
Installed Using : VICIbox 3.1.9 64 bit ISO
Install Reference : VICIBox Cluster and Walkthrough Guide
Asterisk version : 1.4.39.1-vici
Vicidial Version : 2.4-299
Vicidial Build : 110214-0001
Above server is only Web/Telephony server in the set-up. No additional software's installed on both servers except ISO install
The set-up is being used for INBOUND calls handling. everything is working fine except when i try to load leads in a list that has custom fields defined i am faced with problem. After selecting the file to upload and selecting custom layout when submit is pressed the next screen doesnt show any fields mapping just Two buttons "Ok to process" and "start over". The problem persists in 3rdGen and Old leads loader. I have tried CSV, Tab Delimited, extension as TXT but no success.
However if I create a list without any custom fields and try leads loading everything works perfectly fine.
Two error messages in apache error logs are included below for your kind reference
