Could you please give us some steps as to how to set up a multi server/load balancing environment with Vicibox?

Most of it should be automatic, just add the server to the servers table, and the associated vicidial_conferences entries, then add the phones and carrier trunks. After you do that the configs should automatically generate for multi-server load balancing from server to server.

Should Vicibox therefore be loaded on every machine?

You could load ViciBox on every machine and then just disable what you don't want to run.
