Agent login to conference with other agent and caller in it

Okay I am getting some weird problems with a vicibox install that is being used for predictive dialing.

Sometimes an agent will get kicked off the system while waiting for a call, then the agent logs back in and directly into a conference with another agent already logged into it. Sometimes that agent already has a customer on the phone. I can't figure out why this is happening it seems to come and go without any reason. It will happen across multiple campaigns and within the same campaigns.
Another problem I am getting is sometimes an agent will be logged in waiting for a call and 2 calls will come in at the same time. So for example I had an agent a moment ago receive 2 answering machines at the same time, I had the agent log out and log back in and it seems to be fine but this is happening from time to time.
Anyone else have these odd problems? Is it something with my configuration? Any advice would be appreciated.
