asterisk / vicidial dropping multiple calls to agents

Hi I need, to know why my agents are getting two calls dropping in at once and/or why some calls they can not hung up a caller.
I have searched google and found no answers for this happening, and wondering if anyone else has seen this happen?

We have two vicidial servers (one is currently in the proccess of getting another CPU added to it)
Main vicidial with the issues - Vicidial 1:
Vicidial - VERSION: 2.2.0-234 / BUILD: 100116-0718
Asterisk 1.4.26


Vicidial 2 - Has issues when we have 40-48 people on it.
Vicidial - VERSION: 2.4-309a - BUILD: 110430-1642
Asterisk 1.4.39.1-vici

First I would recommend upgrading to at least version 2.7 RC1. If that doesn't fix the problem it is usually caused by an overloaded server or networking issues.

there might be an other possible reason too, double clicking in the agent interface:
