I note you did not post your precise (step by step) method of re-creating the issue.
You should note that many posts have stated "same issue" and then found out that their problem was unrelated except for this shared symptom.
It can happen if your dialplan entry is not right. It can happen if you use the override option on manual dial ... all sorts of errors can cause this symptom (does your Admin->Server-> Asterisk version match what version asterisk states on startup of "asterisk -R"?).
So ... share your step-by-step calling method with the problem moment described clearly. You could also post your dialplan entry (from your carrier) and a CLI of a sample failed call (not 3000 lines of unrelated code, mind you, just a controlled situation with just this one call in the CLI to prove your point).
Then we can see which of the likely problems happens to fit your scenario and get you going. Not that it's impossible that it's a bug ... but it doesn't seem to work out that way usually.
