Call Rejected: Congestion Cause 21

Hey everyone,

I am getting the following error message when manual dialing numbers:
Dial Alert:
Call Rejected: Congestion
Cause: 21 - Call Rejected

Here is asterisk output as well. Captured it the best I could:

When I try to run the auto-dial on the campaign, no calls go through either. I can take inbound calls, just not make outbound. Here are my carrier settings as well just in case:
