Same thing here...been pulling my hair out trying to isolate or identify a fix.
We also have Dial Tones on some (Sporadic) Manual Dial Calls...raloheni have you experienced this as well? The Ring is a "PSTN" normal ring, not the ring we have set in Vicidial...which does kind of point to carrier...
Started happening to us beginning of October, posted it here:
http://forum.eflo.net/VICIDIALforum/viewtopic.php?f=4&t=33570 but got the same reply, "carrier" , it is a tricky one as other than getting Agents to write down numbers (killing time on the phone), sending PCAPs off to the carrier (which appear to be 200 ok completed calls...the ringing ones), etc...we have made little progress and would love any direction on how to isolate this and come up with a fix or workaround.

Or: Create a new system status called "RING", collect them. Report it, demand a refund for all those fake answers, and demand that the carrier prune his upline to remove whomever caused the offense. You'll be doing him a favor.
We have not experienced this in over 3 years, and now all of a sudden all of our carriers? We are using good carriers as well, not the "cheap" ones. Currently have 7 Carriers which have had a great track record for a few years with us...
I am not an expert in Wireshark, and if anyone could even suggest where to look to point out to the carriers where it is (in a PCAP or log or?), this could be helpful to get them to pull upstream carriers that are causing this...
We didn't do any updates on anything that should have changed things on our side...have played with different Dial Options "HRr" - We have isolated every carrier and experience the same thing...
I am at the point where I am pulling PCAPs on all of them and getting them to the carriers.
