Error loading of leads

I have this error during loading of leads... i used csv and tab delimited... after i chose file, list ID overide, phone code overide, custom layout, duplicate by phone in list ID then submit... the input box for lname, fname, address1, city and etc didn't appear.. only the text below appear..
Then I noticed, ALL the leads from my old campaign are gone..So manual dialing is not possible error is no leads in the hopper..PLS HELP..

When you say you have no leads in an existing list after this it is a big red flag. The list loader will not touch anything already in the list it will only check leads against what is already there. Did you clear out the list or possibly delete it?

the list are all there in the list menu, but the leads loaded in all the campaigns arel gone.. dont know if it was accidentally clear or deleted.. actually one of my people with admin rights in the vici is the one who used the server.. he also cant remember his last command... so when i tried loading again the leads, the part where to set the phone, address1, city, state etc after submit didn't appear on the screen.. so if i click OK to process, invalid phone etc flagged coz the phone address1 is not set in the process..
