1) Is the user in question assigned to the "agent" user group?
2) "The correct permission" is vague. List the field(s) and value(s) which you feel apply to this situation.
3) I originally said to check two things: The user must have permission (which you've partially addressed) and the campaign must be active (which you did not address).
I did forget to mention that you must ensure the proper user and pass are being entered. It's very common to enter the Phone and Phone pass here and of course those will not work.
